Welcome to Pediatric Partners! We are always excited about meeting new patients during their first visit to our office – whether the patient is a newborn, new to our area, visiting, or transferring from another pediatrician. We have some new patient information, forms, and office policies information that we’d like to share with you prior to your first visit with us. Please take the time to complete the forms located below or in our forms and policies section. Also, we ask that you bring your current insurance card with you to your first visit and arrive 15 minutes early.
Additionally, please call to schedule your newborn’s first exam within 72 hours of hospital discharge.
Expecting
Congratulations! Raising a child will be one of the most rewarding experiences of your life. We encourage expecting parents to visit our office, meet one of our pediatricians, and learn about our practice before your child arrives. We offer a prenatal class once a month for our expectant parents. Join other expecting parents in this informative and interactive class that is led by one of our pediatricians or nurse practitioners. Come prepared with your questions. Please call our office today to sign up for a prenatal visit (561-741-0000). There is no charge for this service.
Prenatal Visits

We are striving to be the best medical home for our patients.
What is a Medical Home?
The Medical Home is an innovative, team-based approach to providing health care. A partnership develops between the patient, his or her primary care physician and the health care team. Together, following evidence-based guidelines for medical care, the team will coordinate the services you need to provide the best healthcare possible.
What are the changes you as a patient can anticipate?
Team based Care:
Your primary care provider will work with the rest of the Pediatric Partners team of primary care providers (physicians and nurse practitioners), nurses and support staff. This team will work with you to provide all of your healthcare needs.
Improved Healthcare Access:
You do not need to use urgent care centers to be seen for urgent issues during working hours. One of our primary care providers will see you on the very day you have an urgent healthcare need. You can call ahead and schedule with your primary care provider or you can come to the office during our walk in clinic (7:30-8:45AM M-F in our Jupiter Offices and 8:00-8:45AM in our Boca Raton office) where there is always a physician or nurse practitioner available. Routine and follow-up care will be provided in a timely manner and can be arranged by contacting our team directly. We will be monitoring your medical needs and will proactively contact you to schedule follow-up appointments at a convenient time for you. Our Medical Home Team will help to coordinate your follow-up and specialty appointments as you request.
Improved Communication and Access to Information:
You can communicate with a Medical Home Team member anytime during normal working hours by calling 561.741.0000. We now have a patient portal through our website that will allow you to electronically communicate with your healthcare team, and receive electronic communication regarding issues and concerns.
How will I contact my Medical Home Team?
You can contact your Medical Home Team directly to arrange an appointment or to discuss healthcare needs by calling 561.741.0000 from 7:30AM-6:00PM Monday through Friday (except holidays). you can also use the patient portal to contact us as well. Outside of normal office hours for urgent matters you can call the same number and your call will be answered by one of our providers or the All Children's Hospital Triage Center. If needed, they will contact the on-call doctor to give you a call. Please call 911 for all emergencies. It will be necessary for you to let the Medical Home Team know if you are seen by any other provider or if there are any changes to child's medical history. This will allow us to continue to coordinate your healthcare needs.
